PACE bundles primary care, specialty care, adult day, transportation, meals, and even nursing home placement if needed. For eligible families, it can be life-changing.

Eligibility

  • 55 or older.
  • Live in a PACE service area.
  • Certified as needing nursing-home-level care by your state.
  • Able to live safely in the community at the time of enrollment.

What PACE covers

  • Primary care + most specialists.
  • Adult day program with meals and activities.
  • Prescription medications.
  • Transportation to and from PACE center and medical appointments.
  • Home care, social work, physical/occupational therapy.
  • Hospital and nursing facility care when needed.

Frequently asked questions

Does PACE cost anything?
If your loved one qualifies for both Medicare and Medicaid, usually no monthly premium. Otherwise there's a monthly PACE premium.
